We work alongside your treatment plan, not around it.
Therapists and prescribers
PRP wraps community based rehabilitation around the therapy and medication management you already provide. You keep the clinical relationship.
Hospital discharge planners
Warm handoffs after inpatient or crisis stabilization, with same-week intake so clients do not fall through the gap.
Case managers and agencies
Support with housing stability, benefits, daily living skills, appointment follow through, and family coordination.
Schools and youth programs
Child and adolescent PRP with school coordination, in-home skill building, and caregiver support.
Three steps, no chasing.
1. Send the referral
Use the secure online referral form, or call us for our fax line and direct intake contact.
2. We verify eligibility
Our team confirms diagnosis, treatment relationship, and Medicaid or plan coverage, then contacts the client.
3. Intake is scheduled
Most clients are seen the same week. You get confirmation and, with a release, ongoing progress updates.

What are the eligibility requirements for PRP?+
The client needs a qualifying mental health diagnosis from a licensed provider, a current treatment relationship with a prescriber or therapist, and active Medicaid or an accepted plan. Our intake team verifies eligibility and coordinates any missing pieces.
How fast will my client be contacted?+
We contact referred clients and the referring partner within one business day, and most clients are scheduled for an intake the same week.
What documentation should I send?+
A referral form, current treatment plan or discharge summary, a recent psychiatric evaluation if available, and a copy of the insurance card. Missing documents do not block a referral; we will follow up.
