Understanding Intensive Outpatient Programs (IOP): What to Expect

Open Arms Clinical Team, LCSW, LCPC, PMHNPJuly 5, 20261 min read

An Intensive Outpatient Program, IOP, is a structured, higher intensity form of outpatient care. It gives clients the support of a treatment community while keeping them at home, at work, and connected to their families.

Who benefits from IOP?

  • Adults with moderate to severe substance use disorders who need more than weekly therapy.
  • Clients stepping down from residential, PHP, or detox.
  • People whose recovery has stalled with lower intensity care.
  • Anyone whose life is beginning to unravel because of use but who does not require 24 hour supervision.

What a week of IOP looks like

Most IOPs meet three to five days per week for a total of nine or more hours. At Open Arms, IOP includes:

  • Three group sessions per week, each 90 to 180 minutes.
  • Weekly individual therapy.
  • Weekly medication check ins when MAT is part of the plan.
  • Family education sessions.

IOP vs OP vs residential

LevelHours per weekWhen it fits
OP (Outpatient)1 to 8Mild use, stable environment
IOP9 to 19Moderate to severe use, needs structure
PHP20 to 30Higher acuity, still living at home
Residential24 hour careUnsafe environment, medical needs, failed lower levels

How long does IOP last?

Most clients complete IOP in eight to twelve weeks, then step down to standard outpatient with continued individual and group work.

Will IOP fit around work and family?

Yes, that is the point. Many IOPs offer morning and evening tracks. At Open Arms, our morning IOP starts at 9 AM; evening IOP starts at 6 PM.

Getting started

An intake specialist completes an ASAM level of care assessment to confirm IOP is the right level for you. If a higher or lower level of care fits better, we say so and help you get there.

Some sessions can be virtual. Ask our intake team about the current mix.
